REGISTRATION INFORMATION AND DIRECTIONS FOR FULL YEAR 2008

Complete the REGISTRATION FORM. A complete and up-to-date registration form must be submitted for each player. Sign and date the consent /release statement.

New players must submit proof of age. Please submit a copy of a birth certificate or passport along with the registration form. Children born on or before July 31, 2003 are eligible to play. PLEASE do not request new players to be placed on specific teams or with specific friends. Every attempt is made to form teams by neighborhood. Placement also depends upon coach availability.

PLAYER FEES* Full Year Registration fee:
U11-U5 (8/1/96-7/31/03) $90.00 OR
U19-U12 (8/1/88-7/31/96) $110.00
City of Alexandria non-resident fee: $25.00
Late fee $25.00
UNIFORM* A uniform is required to play in any ASA game and may be purchased at the ASA office during office hours. Shin guards are mandatory at practice and games.
SHIRTS (U5-U11): $ 15.00 (Set of 2 tees: red and blue)
SHORTS: $ 10.00
SOCKS: $ 5.00
SHIN GUARDS: $ 5.00
BALL: $12.00
REFUNDS ASA will not make refunds after the first game of the season has been played unless your child cannot be placed on a team.

Complete the VOLUNTEER FORM if you wish to volunteer. ASA is a volunteer organization in our Alexandria community. All of our coaches, office workers, coordinators and board members are volunteers. Each team needs a head coach and an assistant coach. Each age-group needs a coordinator. This is a recreation league - our children need us to play soccer and have fun!

Return the REGISTRATION FORM, copy of BIRTH CERTIFICATE (if new player), VOLUNTEER FORM and PAYMENT (check, cash or money order) to ASA office by FEBRUARY 7, 2008. Any registration received after 8:00 pm on 02/07/08 at the ASA office will incur a late fee of $25.00. Make all checks payable to ASA. Returned checks will be charged a $25.00 fee.

OFFICE LOCATION: The ASA office is located at 1108 Jefferson St. in the Lee Center. We are on the first floor, first hallway on the left, first office on the left. Deliver forms here, or mail to: ASA, P.O. Box 25996, Alexandria, VA 22313. (Do not mail registrations to the Lee Center address.)
DIRECTIONS: from Rt. 1 South: merge right onto S. HENRY ST., turn right onto FRANKLIN ST., turn left onto FAYETTE ST., and then left onto JEFFERSON ST.
HOURS: Tuesdays, Wednesdays and Thursdays 9:15am-12:15 noon.
* Financial aid is available for both registration and uniform fees. Our aid program is based on the same criteria as the Alexandria City Public School Lunch Program. A copy of your enrollment in the lunch program is required for aid consideration, |
